Enrolment Guide



GENERAL GUIDELINES FOR STUDENTS:

1. Enrolment by PROXY is not allowed.
2. Seek the signature of the concerned officer in each step.
3. Subjects should be taken in correct sequence as specified in the prospectus or course
    curriculum. No credit will be given to a subject with a prerequisite subject not yet taken.
4. The student is considered officially enrolled only after he/she has completed all the steps
    of enrolment and is served the Official Registration Form.
5. The student’s official ID will be issued by the Registrar’s Office at least within the
    first two weeks of classes.


A.  NEW FRESHMEN STUDENTS


PRE-ENROLMENT

STEP 1. AT THE GUIDANCE & TESTING CENTER


For enrollee with incomplete admission credentials, to:
• Accomplish upper portion of Enrolment Guide.
• Fill out Memorandum of Agreement (MOA) at the Registrar’s Office.

Note: For admission under the temporary enrolment status pending his/her submission of
          the following credentials:____Form 138-A;____CGMC_____;BC____

For Enrollee with complete admission credentials, to:
   • Accomplish upper portion of Enrolment Guide.
   • Submit accomplished Cumulative Record.
   • Accomplish Student Profile Form (SPF).
   • Submit the following admission credentials:
   [ ] Form 138-A (High School Card) – original copy & 2 photocopies
   [ ] Certificate of Good Moral Character – original copy & 2 photocopies
   [ ] Photocopy of Birth Certificate in NSO Security Paper – original & 2 photocopies
   [ ] Result of admission test – original copy & 2 photocopies
   [ ] Recently taken 2 x 2 color ID pictures – 3 pcs.
   [ ] NCAE Result (optional)
   • The checked ( v ) admission credentials are attached to the Enrolment Guide.

STEP 2. ADMISSION IN THE PROGRAM IN THE RESPECTIVE COLLEGES

STEP 3. BUSINESS OFFICE FOR DOWN PAYMENT AND OTHER FEES:

STEP 4. SUBMISSION OF ENROLMENT GUIDE & ADMISSION
              CREDENTIALS TO THE COLLEGES


ENROLMENT PROPER

STEP 5. FILLING OUT OF PRELIMINARY ENROLMENT FORM BY THE
              COLLEGE ENROLLING OFFICERS

    • Scheduling of subjects
    • Submission of the Enrolment Guide, Preliminary Enrolment Form and admission
     credentials by the College Dean to the Registrar’s Office.

STEP 6. ENCODING OF SUBJECTS & PRINTING OF REGISTRATION
              FORM AT THE REGISTRAR’S OFFICE



B. FOR TRANSFEREES


PRE-ENROLMENT

STEP 1. At the Guidance & Testing Center
   • Accomplish upper portion of Enrolment Guide.
   • Take admission test (Pay 150.00 at the Business Office).
   • Accomplish Student Profile Form.
   • Accomplish Cumulative Record.
   • Submit the following admission credentials:
   • *[ ] Certificate of Transfer Credential/Honorable Dismissal
     - original copy & 2 photocopies
   • *[ ] Transcript of Records for evaluation purposes - 2 copies
   • *[ ] Certificate of Good Moral Character - original copy & 2 photocopies
   • *[ ] Photocopy of Birth Certificate in NSO Security Paper
     – original copy & 2 photocopies
   • [ ] Recently taken 2 x 2 color ID pictures - 3 pcs.
   • For student with incomplete admission credentials secure Promissory note at the
     Registrar’s Office
   • Go back to Guidance and Testing Center (GTC) and claim slip for orientation schedule.
   • Transferee Orientation


STEP 2. Dean’s Office for Interview /Acceptance and Crediting of
              Major/Professional Subjects taken from previous school(s), (if applicable).

STEP 3. Business Office for Payment of Evaluation Fee.

STEP 4. Registrar’s Office :
   • Present Official Receipt & Official Transcript of Records for Evaluation and Crediting
     of General Education Subjects taken from previous school (s)
   • Submit admission credentials.

STEP 5. Business Office for downpayment and other fees :

   • Scheduling of subjects

STEP 7. Encoding & Printing of Registration form at the Registrar’s Office

STEP 8. Student ID application and picture-taking at Room B-201
